Job description
North Country Healthcare is seeking an experienced Hospice Nurse Practitioner to provide comprehensive palliative and hospice care to patients with terminal illnesses. You will collaborate with the Hospice Medical Director, attending physicians, and the interdisciplinary team to evaluate patients and manage symptoms.
The APRN will develop and revise treatment plans, educate patients and families, and ensure care aligns with CMS Hospice Conditions of Participation and state/federal regulations.
