Job description
St. Peter's Health Partners Community Hospice in the Catskill Region seeks a per diem RN to join the Home Care Team, serving Albany and Schenectady counties. Weekday coverage Monday to Friday 8am–4:30pm, no weekends.
You will coordinate with an interdisciplinary team to develop care plans, supporting patients and families. Requires NY RN license, ADN or BSN preferred, 1 year hospice/home care experience, valid license and vehicle.
