Job description
Sanctuary Hospice is seeking a Per Diem Hospice RN Case Manager (RNCM) to join our dedicated nursing team in Tinley Park. You will deliver hands‑on hospice care, coordinate services, and guide patients and families through every step of the hospice journey with compassion and clinical excellence.
You must be a licensed RN, with at least 1 year in medical‑surgical/acute care, EMR experience, and CPR if required by state. A valid driver’s license and reliable transportation are required.
