Job description
Sanctuary Hospice is seeking a Per Diem Hospice RN Case Manager to join our compassionate nursing team delivering hands-on end-of-life care across Summit, Portage, Medina, Stark, and Wayne counties. You will coordinate services, support patients and families, and work with an interdisciplinary team to ensure comfort and quality of life.
You must hold an active RN license with at least 1 year of medical-surgical/acute care experience; EMR proficiency and CPR certification are required, plus
