Job description
New Jersey, Inc. is seeking a Per Diem RN Case Manager to provide skilled nursing care to hospice patients. This role requires effective collaboration within an interdisciplinary team and the ability to address diverse needs of patients and families.
Ideal candidates should be graduates of accredited nursing programs, possess NJ RN Licensure, and preferably hold a CHPN certification. Flexibility in scheduling with a pay rate up to $47 per hour makes this position appealing.
