Job description
Holisticare Hospice is seeking a Per Diem Hospice RN Case Manager to join our nursing team. You will deliver hands-on hospice care, coordinate services, and guide patients and families through the hospice journey.
Responsibilities include responding to referrals, collaborating with the Clinical Director, providing direct care with compassion, and educating families to participate in care plans. Requires RN license and valid driver's license.
