Job description
Signify Health, part of CVS Health, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations in Sterling, VA and surrounding areas. Visits last about 45 minutes and include a medication and medical history review, a physical examination, and optional tests as ordered by the health plan.
