Job description
Signify Health, part of CVS Health, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to perform In-Home Health Evaluations, engaging with people in their homes and bridging gaps in care.
The role offers part-time hours (12 hours weekly) with a base hourly pay range of $41.85–$90.13, plus incentives and benefits. Travel within licensed states is required as needed.
