Job description
Signify Health, part of CVS Health, seeks a per diem Clinician (NP/PA) to perform in-home health evaluations. You will engage directly with patients in their homes, review medications and history, and conduct physical assessments using a digital workflow.
Flexible to travel locally and within licensed states; part-time hours. Per diem role with a base hourly rate and incentive structure, plus malpractice coverage and supplies provided.
