Job description
Circadia Health in California is seeking a Nurse Practitioner to lead patient care across SNF and Home Health AI programs. You will conduct assessments, respond to predictive signals, and coach facility teams to translate data into proactive interventions that reduce readmissions.
You'll round on new admits and facility-flagged patients in SNFs, and extend high‑quality care into patients' homes. Travel within a 30–40 mile radius is expected, with mileage and lodging covered by policy.
