Per Diem Nurse Practitioner Home Health | Oaktown, Indiana
Embrace a unique opportunity to make a meaningful impact through per diem home health risk assessments in Knox County, IN. This Nurse Practitioner role offers the flexibility to design your own schedule with shifts available any day of the week, supporting Medicare Advantage plan members and enhancing the quality of care in the community. Each visit lasts approximately 45 minutes, focusing on annual wellness checks or post-discharge assessments for individuals residing in Vincennes, Bicknell, Monroe City, and nearby areas.

Minimum weekly or bi-weekly hourly commitments available
Key Responsibilities:
Conduct home-based annual risk assessments for Medicare Advantage members
Review prior and current health histories, existing conditions, and medications
Educate patients on their health conditions and medication regimens
Identify gaps in care and recommend actionable solutions
No responsibility for developing treatment plans or writing prescriptions
Qualifications:
Active Nurse Practitioner license in Indiana
Board Certification (AANP or ANCC)
Graduation from an accredited MSN program
Current CPR certification (BLS)
Minimum of 6 months Nurse Practitioner experience preferred
You’ll receive compensation of $80-$90 per completed assessment, with additional stipends for mileage and recognized administrative time.
