Per Diem Nurse Practitioner Home Health | Potsdam, New York
Exceptional opportunity for a skilled Nurse Practitioner to perform home health risk assessments for Medicare Advantage plan members within St. Lawrence County, New York. This per diem role offers a high degree of flexibility, making it ideal for providers seeking to manage their schedules while making a meaningful impact in the community. Assessments typically take 45 minutes and may involve annual health evaluations or post-discharge visits. Locations include Potsdam, Massena, Ogdensburg, and Canton, with candidates potentially scheduled in any of these areas during their shifts.
Flexible shifts, available Monday through Sunday
Set your own hours and create a personalized schedule
Minimum weekly or bi-weekly hourly commitment as needed
Complete comprehensive in-home health risk assessments for Medicare Advantage members
Review and document prior and current medical history, conditions, and medication use
Educate members on their health conditions and prescribed medications
Identify gaps in care and offer practical recommendations
No prescribing or treatment plan development required
Active Nurse Practitioner license in New York
MSN degree from an accredited nursing program
Current CPR (BLS) certification
Board Certification (AANP or ANCC)
At least six months of NP experience preferred
Earn $80-$90 per completed evaluation, plus a mileage stipend and compensation for administrative time
