Job description
Per Diem Nurse Practitioner - Wapakoneta, Ohio
Enhance patient care in the community by delivering personalized home health risk assessments as an experienced Nurse Practitioner. This per diem opportunity offers flexibility and autonomy while you support Medicare Advantage plan members throughout Shelby County, OH including Sidney, Fort Loramie, and Anna.
Schedule:
Flexible shifts available Monday through Sunday
Self-scheduling to suit your availability
Commitment of minimum weekly or bi-weekly hours
Key Responsibilities:
Complete annual and post-discharge home health risk assessments each lasting approximately 45 minutes
Evaluate patients recent and past medical histories, as well as current medical conditions and medications
Educate individuals about their health conditions and medication regimens
Identify gaps in care and recommend appropriate follow-up steps to resolve them
Focus on assessment no requirements for treatment plans or medication prescribing
Qualifications:
Active Nurse Practitioner License in Ohio
Current CPR (BLS) certification
Board Certification via AANP or ANCC
At least six months of recent NP experience preferred
MSN from an accredited nursing program
