nWhat You’ll Do
nPerform PT evaluations and follow-up visits per physician orders
nDevelop and adjust individualized plans of care
nDeliver therapy for gait, balance, mobility, strength, and fall prevention
nTrain patients and caregivers on home exercises and safe mobility strategies
nDocument per NJ DOH, Medicare, and BrightStar standards
nCollaborate with RNs, OTs, STs, and CHHAs
nWhat We Need
nNJ Physical Therapy license in good standing
n1+ year of PT experience (home health preferred)
nCPR/BLS certification
nReliable transportation within Camden/Cherry Hill
nStrong documentation and patient communication skills
