Human Touch Home Health is looking for compassionate Physical Therapists (PTs) for PRN home health opportunities in Quakertown, Coopersburg, and nearby communities.
If you want the flexibility of PRN without being expected to drive all over the region, this may be the opportunity for you.
Patient opportunities are available around Quakertown and Coopersburg , and you can tell us where you want to work and when you're available.

Patient opportunities may be available in:
Quakertown Area: Quakertown, Richlandtown, Milford Square, Trumbauersville, Perkasie, Sellersville
Coopersburg Area: Coopersburg, Center Valley, Emmaus, Hellertown, and nearby communities
You do NOT have to cover the entire Upper Bucks or Lehigh Valley area.

PRN should actually feel flexible.
What You'll Do
Complete physical therapy evaluations in patients' homes
Develop individualized plans of care
Provide therapeutic exercise, gait, balance, transfer, and mobility training
Help patients improve strength, endurance, mobility, and independence
Assess home safety and identify fall risks
Develop and reinforce home exercise programs
Educate patients, families, and caregivers
Monitor progress and adjust treatment plans as needed
Complete reassessments and discharge planning
Collaborate with the interdisciplinary care team
Complete accurate and timely documentation using WellSky
What We're Looking For
Active Pennsylvania Physical Therapist License Required
Graduate of an accredited Physical Therapy program
Valid Driver's License and reliable transportation
Strong clinical and communication skills
Ability to work independently in patients' homes
Home Health Experience Preferred, but Not Required
WellSky Experience is a Plus
