nGrowing home care agency seeking experienced Per Diem RN to conduct client assessments, supervisory visits, and skilled nursing services in the home.
Responsibilities:
n· Initial assessments & reassessments
n· Develop/update Plans of Care
n· Conduct caregiver supervisory visits
n· Skilled nursing visits as ordered
n· Ensure NJ compliance & timely documentation
Requirements:
n· Active NJ RN License (required)
n· 2+ years clinical experience (home care preferred)
n· Reliable transportation
n· Strong documentation & communication skills
Schedule:
nPer diem / As needed Evenings or weekends a plus
