Job description
Rehabilitation Hospital of Northern Indiana in Mishawaka, IN seeks a PRN Case Manager to join our patient-centered care team.
You will work with physicians, therapists and nurses to develop care plans, monitor progress, and assist with discharge planning for patients recovering from serious injuries or illnesses.
This per diem role offers flexible scheduling and opportunities to collaborate across disciplines in a state-of-the-art rehab facility.
