Job description
Albany Med Health System in Latham, NY is seeking a Registered Nurse for a Per Diem position in the Internal Medicine & Pediatrics Office. You will support case management, hospital follow ups and patient messages, working alongside physicians to ensure high-risk patients receive exceptional care.
The RN triages changes in status, coordinates post-hospital and urgent care follow-ups, and provides patient education on injections and glucometer use. Collaboration with LPNs/MAs is essential.
