Job description
Albany Med Health System seeks a per diem Registered Nurse for our Internal Medicine & Pediatrics Office in Latham, NY. The RN will assist with case management, hospital follow ups, and patient messages, working with physicians to meet care goals.
Responsibilities include triaging changes in status, coordinating post-hospital and ER follow-ups, and educating patients on injections and glucometer use. Collaboration with LPNs/MAs and other staff is essential.
