Job description
LiveWell Homecare Agency, LLC is seeking qualified per diem home health nurses to provide comprehensive patient care in patients' homes. You will perform assessments, manage discharges, and educate patients according to policies and procedures.
Documentation in the Epic system is required, with attention to safety needs and public program instructions to ensure appropriate care. Visits are scheduled as needed, following department guidelines and organizational standards.
