About the Role:
Karing Hearts Home Health is looking for a compassionate and skilled Per Diem RN to join our dedicated home health team in Imperial, CA. This is a fantastic opportunity for a registered nurse who values flexibility while making a meaningful difference in patients' lives right in their own homes. Responsibilities:
Conduct skilled nursing assessments and develop individualized patient care plans in a home health setting
Administer medications, wound care, IV therapy, and other clinical treatments as ordered
Monitor and evaluate patient progress, adjusting care plans as needed
Educate patients and family members on disease management, medications, and self-care techniques
Collaborate with physicians, therapists, and interdisciplinary team members to coordinate patient care
Accurately document clinical notes and patient records in compliance with regulatory standards
Ensure patient safety and comfort while promoting independence and quality of life
Requirements:
Active California Registered Nurse (RN) license in good standing
Minimum 1 year of clinical nursing experience; home health experience preferred
Current CPR/BLS certification required
Valid California driver's license and reliable transportation
Proficiency with electronic medical records (EMR) and clinical documentation
Strong communication, time management, and critical thinking skills
Compassionate, patient-centered approach with the ability to work independently
