Job description
Title: Outpatient Pharmacist
Location: Kingman AZ
Shifts: Shift Info: 5 days a week; 8 hour days, 32-40 hours per week; every other weekend; rotating shifts between 6am and 10pm. , 40 hours weekly
Requirements:
PharmD from ACPE accredited school of pharmacy? Licensed in state of Arizona?
1. General job duties. Types of procedures required.
Responsible for dispensing prescriptions and monitoring patient drug therapy in accordance with all applicable regulations and standards. This responsibility includes activities inclusive of, but not limited to, medication order/prescription dispensing, compounding, monitoring of patient medication therapies, counseling patients, documenting interventions, providing drug information and supervising support personnel.
2. Equipment utilized in the unit/dept. (Is there computerized charting, medication systems, etc.)
QS1; MediTech Expanse; Pyxis CII Safe
3. Required skills and minimum years of experience.
Minimum of 1 year experience as an Outpatient / Retail Pharmacist
4. Specific licensures and certifications/registrations required. (ARRT, CRT, BLS, ACLS, etc)
Active license and in good standing as a pharmacist in the state of Arizona
5. Shift or schedule requirements. (Will the employee ever work alone? Is call required? If so, how often? Are weekends required?)
Employee will work varied shifts; will NOT be working alone; No "call"; will be required to work every 3rd - 4th weekend or as needed.
6. Floating to other units/departments. (If so, to which units/departments?)
