Job description
Aya Healthcare is seeking a Permanent Staff Home Health LVN/LPN to provide compassionate in-home nursing care for diverse patient populations. In this role, you’ll complete skilled nursing visits, administer medications and treatments, monitor chronic conditions, perform wound care, and educate patients and families on care plans. Collaborate closely with an interdisciplinary team, document accurately in EMR systems, and ensure regulatory compliance while delivering high-quality, patient-centered care in a fast-paced, mission-driven environment.
