Job description
Aya Healthcare is seeking a Permanent Staff MSW to provide psychosocial assessments, counseling, discharge planning and resource coordination for patients and families. In this fast-paced, mission-driven role, you’ll collaborate with an interdisciplinary care team to address social determinants of health, support safe transitions of care, and advocate for patient needs. You’ll document in the EMR, maintain regulatory compliance, and contribute to quality initiatives while working in a people-first, innovative environment with opportunities for growth.
