nOrganization: Mosaic

nPosition Schedule: Full-Time | Salaried | Days M-F with flexibility other hours to meet the needs of the department/agency.

nWhat You’ll Do

nAs a key leader in service delivery, you’ll blend hands-on advocacy with strategic coordination:

nnnEmpower & Advocate: Oversee a diverse caseload of up to 22 individuals, creating dynamic, individual-centered programs that support personal growth and independence.

n

nnLead Annual Meetings & Assessments: Coordinate and present key assessments (CFA, POM, SIS), lead annual review meetings, and present before the Human Rights Committee.

n

nnCollaborate for Success: Build strong partnerships with Home Supervisors, families, guardians, and Service Coordinators to seamlessly drive admission dates and maintain regulatory compliance.

n

nnCoach & Innovate: Train direct care staff in behavior modification techniques and help develop innovative service methodologies.

n

nnMaintain High Standards: Monitor the quality of care provided and maintain clear, professional documentation.

nRequirements

nWhat You Bring to the Team

nnnEducation: REQUIRED - Bachelor’s Degree in Psychology, Special Education, or a closely related field

n

nnExperience: At least 3 years of hands-on experience working with individuals with intellectual/developmental disabilities.

n

nnExpertise: Deep knowledge of habilitative services, Intermediate Care Facilities state regulations, and behavior intervention techniques.

n

nnCertifications: QIDP certification (or state requirements), Med Aide 40, CPR/First Aid and Mandt certification required (if you do not have then required to obtain after hire).

n

nnCore Competencies: Exceptional critical thinking, strong problem-solving ability, excellent written communication, and structured time-management skills.
