Foundations for Care is a compassionate provider of Medicaid Waiver services dedicated to helping individuals with developmental and intellectual disabilities live safely and independently within their communities. We are seeking caring, dependable, and motivated Personal Support Workers to join our growing team.
Position Summary
The Personal Support Worker (PSW) provides direct care and support to individuals in their homes and community settings. The ideal candidate is patient, compassionate, reliable, and committed to enhancing the quality of life of those we serve.
Essential Duties and Responsibilities
Assist individuals with activities of daily living (ADLs), including bathing, grooming, dressing, and personal hygiene.
Assist with feeding and meal preparation as needed.
Provide safe lifting, transferring, positioning, and mobility assistance.
Change bed linens and maintain a clean, safe living environment.
Perform light housekeeping duties, including cleaning client areas, laundry, and organizing living spaces.
Assist with toileting and incontinence care.
Support individuals with community activities, appointments, and recreational outings.
Document services provided and report any concerns, incidents, or changes in client condition.
Follow all agency policies, Medicaid Waiver requirements, and health and safety procedures.
Maintain professional and respectful relationships with clients, families, and team members.
Minimum Qualifications
Must be at least 18 years of age.
High School Diploma or GED preferred.
Reliable transportation and a valid driver's license preferred.
Ability to lift, transfer, and assist individuals with mobility needs.
Ability to pass a Florida Level 2 Background Screening.
Excellent communication and interpersonal skills.
Ability to work independently and as part of a team.
Required Certifications & Training
The following certifications may be obtained before hire or completed during the onboarding process as required by Foundations for Care:
CPR Certification
First Aid Certification
HIPAA Training Certification
APD Zero Tolerance Training Certification
HIV/AIDS Training Certification
Direct Care Competency Certification
Physical Requirements
Ability to frequently stand, walk, bend, stoop, and reach.
Ability to lift and transfer individuals safely, with or without assistance.
Ability to perform physical caregiving tasks throughout scheduled shifts.
