n nnAs a Pharmacist you will:    

nnnEvaluate physician medication orders for drug appropriateness, correct dosage, potential drug interactions, and proper route of administration.

n

nnProvide clinical recommendations to physicians and healthcare staff regarding medication therapy changes and optimizations.

n

nnMaintain up-to-date patient/resident profiles, including medical history, lab results, and current medications (prescription, OTC, and home remedies).

n

nnEnsure laboratory tests required for monitoring high-risk medications are appropriately ordered, reviewed, and communicated to the healthcare team.

n

nnCollaborate with physicians, nurses, and other healthcare professionals to provide clinical and technical support.

n

nnDocument all communications and clinical interventions in the patient’s medical record, including interactions with families and caregivers.

n

nnImplement and enforce policies and procedures for the safe handling, dispensing, and clinical oversight/management of medications.

n

nnEnsure compliance with all state and federal regulatory requirements related to pharmacy practice and medication management.

n

nnReport medication-related errors and adverse drug events to the Pharmacy Manager as required.

n

nnEducate and support clinical team and healthcare agency staff on safe and effective medication handling procedures.

n

nnPerform other pharmacy-related tasks and responsibilities as assigned.

n nQualifications

nQualifications   

nRequired:  

nnnBachelor’s Degree in Pharmacy, Pharm D preferred

n

nnOne year experience in long term care desired

n

nnLicense/Certification(s): current and valid pharmacist license in the state of practice

n

nnKnowledge of drug therapy guidelines, medication safety practices, and regulatory standards (CMS, DEA, FDA).

n

nnProficiency in electronic medical records and pharmacy information systems

n

nnStrong clinical judgement, and effective written and verbal communication

n

nn*Position will be posted for a minimum of 7 business day
