How you'll contribute
A Pharmacist who excels in this role:
Coordinate and provide pharmaceutical care for patients, collaborating with interprofessional team to optimize safe, effective medication use.
Review and verify orders, manage drug therapy protocols, educate staff/patients, and support quality and regulatory standards.
Manage drug therapy protocols as part of an interdisciplinary team.
Review, interpret, and evaluate medication orders to identify, prevent, and resolve drug-related problems.
Provide drug information and education to medical staff, healthcare professionals, patients, and pharmacy students.
Document interventions and outcomes for drug therapy quality improvement.
Verify medication orders for accuracy/completeness prior to entry.
Check filled medications prepared by technicians.
Maintain proficiency with pharmacy automated systems and software.
Manage patients in an outpatient anticoagulation clinic, including point-of-care testing, result evaluation, dosing, and scheduling.
Participates in performance improvement and required continuing education.

What we're looking for
Applicants should have a doctorate degree. Additional requirements include:
Current license with the state Board of Pharmacy.
BLS required.
ACLS preferred.
