Job description
This opportunity is for a Pharmacist role focused on Transitions of Care and Ambulatory Care at a 400-bed hospital in the San Fernando Valley region. As a Transitions of Care Pharmacist, you will play a vital role in enhancing patient outcomes by providing medication education and counseling prior to discharge. Your efforts will help ensure patients understand their medication regimens and have refills ready before leaving the hospital, promoting compliance and healthier recoveries through a "meds to beds" program.
Responsibilities:
Educate and counsel patients on medications before hospital discharge
Manage medication refills to support seamless transitions to outpatient care
Collaborate with multidisciplinary teams to optimize medication therapy during patient transitions
Promote adherence to prescribed regimens to reduce hospital readmissions
Support ambulatory care initiatives within a large county health system
Qualifications/Desired Experience: Licensed Pharmacist with the ability to practice in California
New PGY1 residency graduates preferred; non-PGY1 candidates will also be considered
Experience or interest in ambulatory care and transitions of care services
Strong communication skills and patient-centered approach
Ability to work effectively in a fast-paced hospital environment
Location: Glendale, CA (San Fernando Valley region)
Additional Details: This is a contract role within the Los Angeles County Department of Health System, a comprehensive health care network including multiple outpatient clinics and hospitals.
