PHARMACIST I

nDepartment: PHARMACY - INPATIENT

nJob Summary

nThe Pharmacist I - Staff Pharmacist is responsible for performing a range of distributive and operational duties to ensure the safe and effective use of medications within the hospital. This position supports core pharmacy functions, including order verification, medication dispensing, sterile product preparation, and medication inventory management. The Pharmacist I provides drug information to healthcare providers, ensures compliance with all regulatory requirements, and contributes to departmental quality and safety initiatives. This role is primarily based in the central pharmacy and may include limited clinical responsibilities as assigned.

nCore Job Responsibilities

nMedication Verification and Dispensing:

nnnAccurately interprets and verifies medication orders, ensuring appropriateness of dose, route, frequency, and therapeutic indication.

n

nnDispenses medications in accordance with established policies and procedures, maintaining accuracy and efficiency.

n

nnPrepares and labels medications for dispensing, including unit dose medications, IV admixtures, and controlled substances.

n

n nPatient Monitoring:

nnnMonitors patient medication profiles for drug interactions, allergies, and contraindications.

n

nnIdentifies and resolves drug-related problems.

n

nnDrug Information:

n

nnProvides drug information and consultation to physicians, nurses, and other healthcare professionals.

n

nnResearches and answers drug information inquiries.

n

n nSterile Compounding:

nnnAdheres to USP and guidelines for sterile compounding, including proper garbing, aseptic technique, and environmental monitoring.

n

nnPrepares sterile IV admixtures and other compounded sterile products accurately and safely.

n

nnParticipates in the maintenance and cleaning of the cleanroom and compounding equipment.

n

n nCentral Pharmacy Operations:

nnnAssists with inventory management and ordering of pharmaceuticals.

n

nnReceives and stores incoming medications and supplies.

n

nnParticipates in quality improvement initiatives and medication safety programs.

n

nnMaintains a clean and organized work environment.

n

n nOther Responsibilities:

nnnAdheres to all New York State and federal regulations governing pharmacy practice, including those outlined in the Education Law and Part 63 of the Commissioner's Regulations.

n

nnPerform other duties as required.

n

n nEducation/Experience Requirements

nREQUIRED:

nnnDoctor of Pharmacy (PharmD) degree from an accredited college of pharmacy.

n

nnExcellent communication and interpersonal skills.

n

nnStrong attention to detail and accuracy.

n

nnAbility to work independently and as part of a team.

n

n nLicensure/Certification Requirements

nREQUIRED:

nnCurrent pharmacist license in good standing in New York State.

n nPREFERRED:

nnCompletion of ASHP residency program.
