Job description
This opportunity is for a pharmacist role focused on Transitions of Care and Ambulatory Care within a 400-bed hospital located in the San Fernando Valley region. The position supports patients prior to discharge by providing medication education, counseling, and assistance with medication refills to ensure smooth transitions from hospital to home care. This helps improve medication adherence and patient outcomes through a "meds to beds" program.
Responsibilities include:
Educating patients on medication regimens before hospital discharge
Collaborating with healthcare teams to facilitate medication continuity
Managing medication refills to promote compliance and reduce readmissions
Participating in ambulatory care activities aligned with the hospital's healthcare goals
Desired qualifications: Licensed pharmacist with at least a Pharmacy degree
PGY1 residency completion preferred but new graduates without PGY1 will be considered
Experience or interest in transitions of care and patient counseling
Ability to work effectively within a large hospital and outpatient clinic system
Location: Sylmar, CA (San Fernando Valley region)
This is a long-term contract opportunity at an LA County Department of Health hospital, supporting a diverse patient population. The hospital is part of a large health system with multiple clinics and inpatient facilities.
