Staff Pharmacist | Long-Term Care Pharmacy

We are seeking a licensed Pharmacist to join a collaborative pharmacy team supporting patients in long-term care and post-acute care environments. This opportunity offers a more clinical focus than traditional retail pharmacy, with an emphasis on medication therapy review, patient safety, and interdisciplinary collaboration.
What You'll Do
Review and verify medication orders for accuracy and clinical appropriateness
Evaluate dosing, drug interactions, and potential adverse effects
Collaborate with physicians, nurses, and other healthcare professionals to optimize patient care
Monitor medication regimens and make clinical recommendations when appropriate
Support medication safety initiatives and regulatory compliance
Maintain accurate patient and pharmacy records
Participate in compounding and medication dispensing activities as needed

Active Pharmacist license in the state of practice
PharmD or Bachelor's Degree in Pharmacy
