Job Description
Pharmacist - Outpatient
Schedule
Day 4x10-Hour (07:00 - 17:00) or Day 4x10-Hour (09:00 - 19:00), weekdays only.
Assignment Details
Onsite position split between two primary care clinics, seeing 16 patients per day.
Requirements
BCACP board certification and PGY2 Ambulatory Care residency required
4 years of direct patient care that demonstrates medication management
Pharmacy license from state board and BLS certification
Responsibilities Provide clinical leadership for pharmaceutical care in specific areas.
Develop and optimize pharmacotherapeutic regimens through collaborative practice agreements.
Perform drug utilization reviews and monitor for drug interactions and potential adverse events.
Minimize polypharmacy and develop patient medication action plans with self-management goals.
Preceptor for pharmacy students or residents, participating in education.
Engage as a key component of the multidisciplinary team in patient care.
Communicate and present clinical interventions and medication action plans to the patient care team.
Provide drug information, dosing, and monitoring services to practitioners.
Assist in developing Pharmacy and Therapeutics policies, guidelines, and formulary analysis.
Support coordination of safety initiatives and compliance with relevant standards.
