Job description
Overview
We are looking for a Pharmacist to join our team and provide the highest quality of care, reliability, and support to patients, caregivers, and families. In this role, the Pharmacist will be responsible for dispensing medications, providing clinical advice, and delivery services.
Schedule
Sunday – Wednesday (On‑call): 6:00 pm – 10:00 am (just need to come in per on‑call order)
Saturday / Sunday (1–2 weekends per month): 12:00 pm – 4:00 pm rotation (8–16 hrs per month)
Nightly on‑calls 7 days a week

Responsibilities
Provides clinical consultations to patients as well as recommendations to nurses, physicians, and other hospice personnel
Evaluates medication profiles, compliance, and modifications to achieve desired therapeutic outcomes
Answers customer questions and resolves customer issues in a timely manner
Retrieves data in the pharmacy system
Manages medication orders
Ensures proper storage and security conditions for medications
Maintains a productive, orderly, and error‑free work environment
Qualifications
Doctor of Pharmacy Degree or Bachelor of Science Pharmacy Degree
Current and good standing Pharmacist License
Strong communication skills, customer service, and problem‑solving skills
