Job description
Compounding and preparing ordered pharmaceutical preparations
Contacting and conferring physicians if there are any questions or irregularities regarding medication orders
Preparing TPN solutions and Monitoring TPN Patients as required
Mixing of irrigating solutions and intravenous additive solutions as required using aseptic technique with the laminar air flow hood or CAL, or CACI for hazardous medications
Shows competencies in aseptic technique, garbing and gowning, and didactic training in IV and HD preparation
Consulting with physician and nursing staff concerning availability, form, side effects and incompatibilities of currently available drugs
Responsible for the collection of Quality Assurance/Performance Improvement data
Interprets prescription orders and contacts provider or insurer as indicated
Performs medication counseling for patients as requested
Supervises activities and checks work of pharmacy technicians
Performs other duties as assigned

Education and Experience
Bachelors Degree or Doctorate of Pharmacy, must meet state requirements for Pharmacist and registered by the Louisiana Board of Pharmacy
Minimum of one year experience in hospital Pharmacy preferred
