Job description

The Pharmacy Division is dedicated to ensuring safe, effective, and cost-conscious medication use while collaborating with healthcare professionals to achieve optimal drug therapy outcomes. The Ideal candidate is a licensed pharmacist with a Pharm.D. or Bachelor's degree in Pharmacy from an accredited college. They demonstrate strong clinical knowledge and a commitment to patient-centered care in an ambulatory oncology setting. While hospital pharmacy experience (6 months or more) is preferred, the candidate should excel in accuracy, communication, and collaboration within a multidisciplinary team.

Working Hours: The working hours are 9:30am-6pm Mon-Fri

Job Competencies

Key Responsibilities

1. Communication & Patient Engagement Demonstrate strong interpersonal and customer service skills by treating patients with compassion, dignity, and respect.

Maintain professional and appropriate communication, including language, tone, and volume, when interacting with patients, family members, healthcare providers, and other stakeholders.

Communicate promptly with providers regarding prescription changes, patient concerns, medication-related issues, or other clinical needs as appropriate.

2. Clinical Assessment & Patient Counseling Conduct comprehensive patient assessments, including: Review and maintenance of an updated home medication list.

Assessment of medication adherence.

Evaluation of patient quality of life and therapy outcomes.

Assess the appropriateness of specialty medications and monitor patients throughout therapy to support achievement of desired clinical outcomes.

Provide specialty medication counseling and education, including: Medication administration.

Expected therapeutic outcomes.

Potential adverse effects and their management.

Participate in specialty clinical consultations and collaborate with healthcare providers to optimize patient care.

3. Documentation & Regulatory Compliance Accurately document current and relevant patient information related to specialty medication therapy within the electronic health record (EHR).

Complete and maintain initial patient assessments and ongoing reassessments in compliance with organizational policies and accreditation standards, including URAC requirements.

Maintain thorough and accurate documentation of all patient interactions, including but not limited to: Telephone communications

Emails

In-person pharmacy visits

Clinical consultations

Ensure all documentation is timely, complete, and compliant with applicable regulatory and accreditation standards.

4. Additional Responsibilities Perform other duties and responsibilities as assigned to support departmental and organizational objectives.

Education Required: Bachelor's Degree Pharmacy. or

Required: Pharm.D. degree from an accredited college of pharmacy.

Work Experience Preferred: Six months experience in a hospital pharmacy.

Licenses and Certification Required: RPH - Registered Pharmacist Texas State Board of Pharmacy. within 90 Days and

Required: BLS - Basic Life Support American Heart Association (AHA). within 30 Days and

Required: ACLS - Adv Cardiac Life Support American Heart Association (AHA). If working in one of the following departments: Pharmacy Inpatient ), Pharmacy OP ACB ), Pharmacy-West Houston ), Pharmacy-The Woodlands ), Pharmacy-NWH Sur&SP Care ). within 90 Days

This position may be responsible for maintaining the security and integrity of critical infrastructure, as defined in Section 113.001(2) of the Texas Business and Commerce Code and therefore may require routine reviews and screening. The ability to satisfy and maintain all requirements necessary to ensure the continued security and integrity of such infrastructure is a condition of hire and continued employment.
