Job description
Licensed Pharmacist Contract Position
We're hiring a licensed pharmacist for a contract position where you'll use your clinical expertise to ensure the safe and thoughtful distribution of medications to individuals navigating a range of physical and mental health challenges.
Minimum Requirements
Must have State License to Apply
EPIC experience
Experience with medication reconciliation for discharge medication lists
Communication skills for gathering prior-to-admission home medication lists from patient, family, or pharmacy
Communication skills for discharge counseling with patients and families
Clinical knowledge of antimicrobial stewardship, including interpretation of culture results for recommendations
Clinical knowledge of pharmacokinetic dosing management and monitoring
Clinical knowledge of anticoagulation dosing management and monitoring
