Job description
We're hiring a licensed pharmacist for a contract position where you'll use your clinical expertise to ensure the safe and thoughtful distribution of medications to individuals navigating a range of physical and mental health challenges. Minimum Requirements:
Must have State License to Apply
6 months Pharmacist experience preferred
EPIC experience
Experience with medication reconciliation for discharge medication list
Communication skills for gathering prior to admission home medication list from patient, family, or pharmacy
Communication skills for discharge counseling with patients/families
Clinical knowledge of antimicrobial stewardship, including interpretation of culture results for recommendations and knowledge of UTI, pneumonia, and SSTI for recommendations
Experience with pharmacokinetic dosing management and monitoring
Experience with anticoagulation dosing management and monitoring
Assignment Details Schedule: 5x8
