Job description
We're hiring a licensed pharmacist for a contract position where you’ll use your clinical expertise to ensure the safe and thoughtful distribution of medications to individuals navigating a range of physical and mental health challenges. Minimum Requirements:
Must have State License to Apply
EPIC experience
Experience with medication reconciliation for discharge medication lists
Communication skills for gathering prior-to-admission home medication lists from patients, families, or pharmacies
Communication skills for discharge counseling with patients and families
Clinical knowledge of antimicrobial stewardship
Experience interpreting culture results for recommendations
Knowledge of UTI, pneumonia, and SSTI for recommendations
Experience with pharmacokinetic dosing management and monitoring
Experience with anticoagulation dosing management and monitoring
Assignment Details:
Schedule: 5x8
Assignment Length: 13 weeks
