Verify and fills orders for medications, monitors patient medication therapies, provide drug information, identifies and troubleshoots potential medication safety hazards, documents all interactions for medical review, and supervises and directs support personnel.
requirement :
Variable Shift - 4 x 10s, Mostly Nights 8:30pm to 6:30am. on occasion day shift would be 10:15am to 8:45pm. Rotating days, but generally 8 on 6 off, including weekends. 1 year inpatient pharmacy experience or more required. Christmas Eve and Day coverage can be expected to work. No Call. Must have EPIC experience.Education and Work Experience:Graduate of school of pharmacy: RequiredDoctor of Pharmacy (PharmD) degree: PreferredOne year's technical experience: PreferredInpatient clinical pharmacy experience: PreferredLicenses/Certifications:Pharmacist license in the state of practice: RequiredAll licensures and certifications needed to fulfill the duties required of a Pharmacist at the state level: RequiredFacility Specific License/Certifications:Advanced Cardiovascular?Life Support (ACLS)?certification from approved vendor per AH policy: RequiredBasic Life Support (BLS)?certification from approved vendor per AH policy: RequiredPediatric Advanced Life Support (PALS)?certification from approved vendor per AH policy: Required
