Job description
Fortis Care is seeking a Physiatry Advanced Practice Provider to round at our Savannah partner SNF, focusing on residents who struggle with knee pain, spasticity, and neuropathic pain that hinder therapy participation.
You will assess barriers, perform injections and spasticity management, and collaborate with therapy and nursing to develop durable plans that transfer gains to daily care. Independence and clinical leadership are valued.
