Job description
What You'll Be Doing
nYou are the reason a resident who couldn't tolerate 30 minutes of PT last week is walking to the dining room this week.
nAs Fortis Care's Physiatry Advanced Practice Provider, you round at our partner skilled nursing facility in Savannah and see the residents who are stuck — the ones whose knee pain, shoulder impingement, spasticity, or neuropathic pain is keeping them from participating in therapy. You assess what's blocking them, you treat it, and you hand them back to the therapy team ready to work.
nKey Responsibilities
nn Evaluate residents flagged by therapy or nursing as unable to progress — a focused functional, musculoskeletal, and neuromuscular exam to find the actual barrier.
n Treat it directly: peripheral joint and soft-tissue injections (knees, shoulders, hips), spasticity management including botulinum toxin, trigger point and dry needling, and targeted medication management.
n Build the plan with PT/OT, nursing, and the attending team — including DME, bracing, and adaptive equipment — so the functional gain sticks after you leave the room
n Follow through : recheck, adjust, document, and escalate or refer anything outside physiatry's lane. **
n**
n Teach : you become the facility's go-to resource on therapy readiness, mobility, fall prevention, and pain that's getting in the way of recovery.
n nWho Thrives Here
nn You want to own a panel and be the expert in the building, not the seventh provider in a rotation.
n You're interested in procedures — or already doing them — and want to build real volume in peripheral joint injections and spasticity management.
n You like geriatric and post-acute medicine and find complexity interesting rather than exhausting.
n You're comfortable making decisions independently and know when to pick up the phone.
n You want measurable wins : a resident who regains transfers, a discharge home instead of long-term care, a readmission that didn't happen.
n nSkills & Abilities
nnStrong clinical knowledge of adult and geriatric medicine, with expertise in post-surgical, post-acute, and chronic disease management.
nExcellent communication and collaboration skills, with the poise to manage high expectations from discerning patients, families, and referring physicians.
nAbility to manage a patient panel efficiently while maintaining a high standard of care and an unhurried bedside manner.
n nQualifications
nnCurrent Nurse Practitioner or Physician Assistant license in the state of Georgia.
nFor NPs: certification in Internal Medicine, Geriatrics, or Family Practice required. For PAs: current NCCPA certification required.
nMust have an unrestricted DEA license or wiling to obtain (we reimburse you)
nMinimum 1–2 years of NP or PA experience preferred; experience in subacute rehab, post-acute, SNF, or geriatrics strongly preferred.
nAbility to work independently in a facility setting with physician support.
nFamiliarity with EHR systems and clinical documentation best practices.
n nBenefits Package
nnFlexible schedule that you set
nNo nights, no weekends, and no call coverage
n4 weeks of Paid Time Off (PTO) plus paid holidays
nHealthcare reimbursement account to cover medical, vision, and dental insurance or other qualified medical expenses
n$1,000 CME or licensing renewal
n401(k) Plan after first year of employment
nPaid malpractice coverage ($1M/$3M) including tail coverage
nPaid hands-on procedural training in joint injections, spasticity management and physiatry workflows
nOngoing mentorship from our physician-led clinical team, plus peer case review with other Fortis physiatry providers
n n**Why Fortis Care
n**

nAs part of Fortis Care, you'll join a collaborative, mission-driven team that values independence, ownership, and doing the right thing for patients and partners. We invest in our clinicians, celebrate hard work, and provide opportunities to grow alongside a company that is redefining post-acute care.
