Job description
Physiatrist Opportunity at Iconic Care
As a physiatrist, you will round at three skilled nursing facilities or similar rehabilitation centers, five days per week. Your primary focus will be delivering specialized physiatry care to 25+ patients per day who are experiencing difficulty participating in therapy due to pain or functional limitations. This role commutes to one facility in Tracy two days per week and rounds two facilities in Antioch three days per week.
Key responsibilities include:
Performing focused assessments and developing individualized treatment plans in collaboration with facility primary care teams to optimize therapy participation and functional recovery.
Providing targeted interventions such as joint injections, Botox treatments, dry needling, and medication management based on patient-specific needs.
Serving as the sole physiatrist on-site, operating with a high level of autonomy while supported by a collaborative clinical team for consultation as needed.
Coordinating care and referring to other specialties or services for issues beyond the scope of physiatry.
This role is best suited for a self-motivated and confident clinician who thrives in independent practice environments and is passionate about improving rehabilitation outcomes.
Qualifications:
Current, unrestricted PA or NP license.
DEA licensed or willing to obtain DEA licensure.
Nursing home and/or physiatry experience is a plus, but not required.
Comfort and interest in performing peripheral joint injections (knees, shoulders, hips).
A genuine passion for providing professional, personalized care in an autonomous environment.
Strong communication skills, with the ability to collaborate with multidisciplinary teams.
Familiarity with EHR charting
