About the Role
This is a Full-Time position in Evergreen, Colorado with a Monday-Friday workweek from 8:30AM – 5:00PM. As a Physical Therapist, you’ll be a vital member of our Home Health team, providing compassionate, high-quality care throughout our mountain service area.
In this role, you will:
Provide skilled, home-based physical therapy to patients
Develop and adjust individualized care plans with measurable goals
Educate patients and caregivers on recovery, safety, and long-term wellness
Collaborate closely with RNs, Social Workers, Home Health Aides, and Leadership to deliver holistic, patient-centered care
Primary service area would be the Foothills area of Jefferson County.
Maintain timely, accurate clinical documentation
Daily travel throughout our mountain service area (this is a field-based role)

What You Bring
Required:
PT or DPT degree
Minimum 2 years of experience
Strong clinical, communication, and time management skills
Ability to work independently while contributing to a team
Valid driver’s license, reliable vehicle, and insurance
Legal authorization to work in the U.S.
COVID-19 vaccination (or approved exemption)
Preferred:
Home Health experience
