nSCHEDULE: Day shift, 4 days per week, 10-hour shifts, Sunday - Wednesday
nSUMMARY
nAs a Physical Therapist, you welcome patients and put them at ease. You provide PT to patients with care and compassion to ensure rehabilitation success.
nPlans, organizes, and implements specific treatment programs for individual patients according to the principles and practices of physical therapy. Collaborates with interdisciplinary treatment team. Supervises the work of support personnel.

nRESPONSIBILITIES:
nnnPatient Evaluation. Assess patient impairment and current condition to evaluate rehabilitation potential; report on patient progress, communicate effectively with physician and other team members; counsel patient and family according to degree of disability or impairment

nnTreatment & Planning. Develop physical therapy treatment project for each patient, including training in functional mobility, use of assistive devices, therapeutic exercise, balance and coordination retraining, massage, heat, and cryotherapy; apply diagnostic and progressive functionality tests

nnLeadership. Train, mentor, and coach team members to ensure standards of the department are met; address scheduling needs to ensure proper coverage is provided

n nREQUIRED QUALIFICATIONS:
nnnDoctorate or Master’s Degree in Physical Therapy, or Bachelor’s degree with prior experience as a licensed Physical Therapist required
n
nnFOR ElderONE EMPLOYMENT: 1 year of experience with the frail elderly population preferred, and availability of a motor vehicle.
n
nnRequired Licensure/Certification Skills:
n
n n• Current New York State license or a limited permit as a Registered Physical Therapist • If a New York State license or limited permit is not held, the applicant must be hired within 90 days of graduation from a New York State or APTA-accredited physical therapist education program and meet Practice Exemption under New York State Physical Therapy License Requirements. • If working in Homecare or traveling between locations, a NYS license is required.
nPREFERRED QUALIFICATIONS:
nnPrior Physical Therapist experience preferred.

nPHYSICAL REQUIREMENTS:
nM - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

nThe listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
