Our mission is to help seniors and kupuna improve mobility, reduce fall risks, and maintain independence while receiving care in the comfort of their homes.We are seeking licensed PTs and PTAs who enjoy building strong patient relationships and delivering personalized therapy services.ResponsibilitiesProvide skilled in-home physical therapy servicesComplete evaluations, reassessments, and fall risk assessments (PT)Implement individualized treatment plans to improve mobility, balance, strength, and safetyEducate patients and caregivers on home exercise programs and fall preventionDocument accurately and communicate patient progress with the interdisciplinary care teamMaintain compliance with Medicare home health documentation requirementsRequirementsCurrent Hawaii license as a Physical Therapist (PT) or Physical Therapist Assistant (PTA)Minimum one year of clinical experience (home health experience preferred)Strong documentation, communication, and time-management skillsAbility to work independently in the fieldValid driver's license and reliable transportationPassion for serving seniors, kupuna, and local familiesWhy Join Our Team?Flexible schedulingCompetitive per-visit compensationSupportive local leadershipMeaningful work that makes a difference in the communityGrowing agency with long-term opportunities
