Physical Therapist Assistant (PTA)Sierra Home Health in Truth or Consequences is seeking a Physical Therapist Assistant (PTA) to join our compassionate and collaborative home health team. As a valued member of our tight-knit staff, you'll enjoy flexible scheduling, supportive leadership, and a high level of autonomy — all while serving your local community with integrity.Key Responsibilities:Deliver in-home physical therapy services under the supervision of a licensed Physical TherapistWillingness to travel to Truth or Consequences one to two days a week with mileage compensationCarry out treatment plans to help patients regain mobility, function, and independenceCommunicate progress and changes in patient condition to the supervising therapistEducate patients and caregivers on exercises and safety practicesDocument visits and progress notes accurately and on timeParticipate in care team meetings and interdisciplinary planningUphold infection control and safety standards during all home visitsMaintain professional, compassionate rapport with patients and familiesWhat We're Looking For:Current licensure as a Physical Therapist Assistant in New MexicoPrevious experience in home health preferred, but not requiredStrong interpersonal and communication skillsConfidence working independently in the fieldAdaptability to changing schedules and home environmentsCommitment to quality, safety, and patient-centered careReliable transportation and valid driver's licenseSierra Home Health is part of a nonprofit healthcare organization managed by CareM.
