nSummary:
nAssists with the implementation of physical therapy services that identifies, prevents, remediates and rehabilitates acute or prolonged physical dysfunction or pain in the patient's home. Under the supervision of a physical therapist, delivers treatment including therapeutic exercise, physical modalities, patient and family education and procedures. Work is performed in the patient's place of residence.
nResponsibilities:
nnnWorks under the direction and supervision of the licensed Physical Therapist in the home environment practice setting.
n
nnPerforms Physical Therapy procedures ad related tasks under the direction of the supervising Physical Therapist.
n
nnProvides patient/family education and training.
n
nnPerforms routine operational functions, including superision of Home Health Aide.
n
nnDocuments visits and completes documentation within 24 hours of visit and within agency guidelines.
n
nnMay modify specific treatment procedure in accordancewith patient status within the scope of the established treatment plan.
n
nnParticipates in Performance Improvement activities and peer review.
n
nnCompletes continuing education as per agency and state licensing requirements.
n
nnFulfills all compliance responsibilities related to the position.
n
nnPerforms other duties as assigned.

n nEducation: ASSOCIATE'S LVL DGRE
nOther Information:
nRequired: Customer relation skills with patients and staff. All PT modalities, use of a computer.
nMinimum Experience: zero to six months
nDesired: FIM certification desired.
nWorking Conditions:
