Job description
Physical Therapist Assistant (PTA)At Haven Home Health & Hospice, we believe care should always be about the person, not the profit.As a locally owned organization, we have the opportunity to make decisions with our clients, their families, and our community in mind. We are looking for a Physical Therapist Assistant who shares that mindset and wants to make a meaningful difference in the lives of the people they serve.As a PTA at Haven, you will work alongside a collaborative team of therapists and clinicians to provide skilled physical therapy services that help clients remain safe, independent, and active in their own homes. You will build relationships with clients and families while helping them work toward the goals outlined in their individualized plan of care.What You'll DoProvide skilled physical therapy services under the supervision of a Physical Therapist.Implement individualized plans of care based on physician orders and client needs.Monitor client progress and communicate changes with the supervising PT and care team.Provide preventive and rehabilitative therapy interventions.Educate clients and families on self-management, safety, and physical therapy needs.Maintain accurate and timely clinical documentation.Collaborate with other Haven clinicians to provide coordinated, client-centered care.Participate in department meetings, in-services, clinical outcomes, and performance improvement initiatives.Uphold Haven policies, professional standards, and applicable regulatory requirements.What We're Looking ForCurrent Missouri PTA license in good standingAt least one year of PTA experience within the last three yearsStrong communication and organizational skillsAbility to build meaningful relationships with clients, families, and fellow team membersStrong clinical judgment and ability to work independentlyComfortable with technology and electronic documentationDependable transportation and a current driver's licenseFlexibility to work weekends and holidays as neededWhy Haven?At Haven, people come first.
