You benefit from a cohesive, in-house therapy team and supportive organization with opportunities to learn and grow.Optage is the Home & Community based division of PHS and is seeking a Full Time Physical Therapist Assistant serving in a home health care environment in our communities in the south Twin Cities metro area.$1,000 Hiring BonusSCHEDULE/HOURS: Monday - FridayWHY BECOME A THERAPIST WITH PHS?Great work/life balance and flexible schedulesCompetitive pay and annual increasesUse your strengths to serve multiple settings – TCU/SNF, Assisted Living, Independent Living, Home Health, and OutpatientSpend more time with your residents/clients with reasonable productivity expectationsPaid mileage and drive time if traveling between communitiesAccess to online continuing education courses & podcastsScholarship program – awards up to $5,250 per year!May be eligible for Public Service Loan ForgivenessOpportunities for growth and internal promotion!ResponsibilitiesAs a Physical Therapist Assistant you will be responsible to:Provide treatment per the established plan of care and in accordance with physician orders.Assist in identifying goals, prioritizing treatment, and selecting appropriate interventions.Continuously assess changes in functional deficits and adjust treatment as appropriate.Provide education to residents/clients, families, caregivers, and staff.Collaborate with therapist on observations, progress towards goals, and recommended updates to plan of care and discharge.Provide accurate and timely documentation & billing in electronic medical record.Demonstrate understanding and knowledge of payer sources and regulatory requirements.Coordinate with therapist on supervisory visits per regulations.Communicate effectively with residents/clients, families, caregivers, physicians, referral sources, and interdisciplinary team.QualificationsGraduate from an accredited Physical Therapist Assistant program.Current licensure in the state(s) of practice as required.Demonstrated compatibility with PHS's mission and operating philosophies.Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.Pay RangeThe estimated starting pay range for this role is $30.24 - $34.57 per hour. This good faith estimate is based upon PHS's typical starting pay range for this role.
